Anaemia Flashcards
Causes of microcytic anaemia
Iron deficiency
Anaemia of chronic disease (can also be normocytic)
Sideroblastic anaemia
Thalassaemia
Iron study findings in iron deficiency
Serum iron - low
%saturation of transferrin - low
Ferritin - low
Total iron binding capacity - high
Iron study findings in anaemia of chronic disease
Serum iron - low
%saturation - low
Ferritin - high (hepcidin sequesters iron in liver)
TIBC - low
Causes of normocytic anaemia
Anaemia of chronic disease (can be normocytic)
Haemolytic anaemia
In haemolytic anaemia, what will the reticulocyte count be?
Raised
What causes megaloblastic haemolytic anaemia?
Anything causing impaired DNA synthesis:
Folate/B12 deficiency
What drug can cause megaloblastic anaemia and why?
Methotrexate
Folate antagonist
What is pernicious anaemia?
Autoimmune destruction of gastric parietal cells, leading to lack of intrinsic factor so B12 not absorbed
Causes of non-megaloblastic macrocytic anaemia?
Alcoholism
Hypothyroidism
Reticulocytosis
Drugs (fluorouracil)
Pregnancy
What else can be seen on blood film in megaloblastic anaemia?
Hypersegmented neutrophils
What haemoglobin is the blood transfusion threshold?
70g/l
What haemoglobin is the blood transfusion threshold in patients with ACS?
80g/l
Haemochromatosis iron studies findings
Transferrin saturations >45%
Elevated ferritin
