Radiology M4 - R2 : Quality assurance Flashcards
introducing simple systems, sop
improving working environment
and encouraging reporting without blame
the above would help with what?
help reduce human error
what governs quality assurance
legislations such as;
IRR 2017
IR(ME)R 2017
there are four people responsible for quality assurance who are they?
employer - the boss
practioner - justification
operator - anyone involved in the production of a radiograph
referrer - one who refers the patient for radiograpghs
what do we look at in a quality assurance programme?
image quality
X-ray equipment
processing (film and digital)
working procedures
training
audit
what two things are done to ensure high quality images
audit the images we produce by a
- image quality rating system
- film reject analysis
radiograpghs are graded by image quality, what is the grading system
the grading system is
1 = perfect
2 = not perfect, but can be used clinically
3 = must be re-done
no errors of patient preparation, exposure, positioning, processing or film handling
teh above best describes which quality grade
1
diagnostically acceptable - some errors in patient preparation, exposure, positioning, processing or handling which do not detract from the diagnostic utility of the radiograpgh
the above describes which grading rating
2
unacceptible - errors of patient preparation, exposure, positioning, processing and/or handling which render the radiograph diagnostically unacceptable
the above best describes which image quality grade
3
what are the desired targets for films rated
number of films rated 1 > 70%
number of films rated 2 < 20%
number of films rated 3 < 10%
how would you go abouts doing a reject film analysis
collect all rating/grade 3 films
at end of each month assess these films
categorise faults e.g. exposure, positioning, processing, etc
repeat every month
what are the four cycles of the IQRS audit cycle
assess - evaluate images
grade - 1,2,3
results - analyse look for ways to improve
feedback
Legistlation requires auditing of the X-ray equipment, what is required for this audit.
equipment inventory must be kept
all equipment must be tested regularly to check that it is working satisfactory
preferably annually but minimum of every 3 years
these checks must include representative patient doses
routine maintenance - annually
in reference to film storage why is it important to avoid direct sunlight and UV when it comes to digital PSP
the PSP is sensitive to light and would excite the PSP, which when used would make the image not clear.
it is important that if they are not being used, they should be erased.
what is a sensitometry
compares film processed with fresh chemistry with images produced as developer gets exhausted
exhausted developer = pale image
