The implementation and monitoring of STD 2 HSE event reporting and management, requires the combination of HSE event Management, leading indicator reports and accurate data entry on a monthly basis to provide the information required to enable benchmarking of performance.

- HSE Event Reporting and notification
- HSE event classification
- HSE event investigation, review closure and communication
- Reporting on QUEST of Montly SafetyNET data for benchmarking performance.

1) Brief description of the event (type and severity)
2) GeoMarket, Country and name of the location, rig, vessel, crew, plant, etc.
3) Date and time of occurrence
4) Name of injured persons, if any, including third party

What is the criteria for Major on $ losss, NPT hrs or LWDC and when need to be reported on QUEST?
SQ/PQ ≥ 100K < 1,000K $Loss; ≥ 24 <48 NPT(hr); QUEST 48hrs.
HSE ≥ 100K < 1,000K $Loss; ≥ 100 days LWDC or RWDC (lost day work case or restricted work day case) ; QUEST 48hrs. LM 12 hrs.
What is the criteria for Serious on $ losss, NPT hrs or LWDC and when need to be reported on QUEST?
SQ/PQ ≥ 10K < 100K $Loss; ≥ 4 <24 NPT(hr); QUEST 48hrs.
HSE ≥ 10K < 100K $Loss; ≥ 1 < 100 day; QUEST 48hrs. LM 24 hrs
What is the criteria for Light on $ losss, NPT hrs or LWDC and when need to be reported on QUEST?
SQ/PQ < 10K $Loss < 4 NPT(hr), QUEST 72hrs.
HSE < 10K $Loss; < 1 day LWDC; QUEST 48hrs. LM 24 hrs
