What LCM stand for?
Loss Causation Model
What is LCM?
A mythology to help analyze and understand why events occur.
Systematic analysis tool to identify the HSE Management system failures which enable accidents, near accidents or event to occur.
The Loss Causation Model, shows how a lack of control can lead to a sequence of events that eventually leads to a loss.
Lack of control-> Root Causes-> Immediate Causes -> HSE event -> Loss
LCM model principles?
- Accidents don’t just happen.
- Multiple causes usually contribute.
- Fixing immediate causes is not prevention, and root (system) causes must be identified.
- Root (system) causes are created by a lack of management system control.
- Plans of action must correct the root causes and lack of controls.

Driving, HAZMAT (chemicals), High pressure, New employees, Potential energy, weather (Extreme cold/hot temperature)
Describe SLB Involved:
Any HSE event associated with a SLB related activity.
A Schlumberger related activity is any work-related activity, plus any activity:
- Taking place at any SLB controlled facility
- Involving an SLB vehicle
- Involving any private vehicle or taxi on SLB business
- Involving any client or contractor activity related to SLB
- Involving the use or access to SLB computer or networks
- That is company sports and taking place off site where management control is expected
Describe SLB Concerned
All SLB nonevolved HSE events concerning company employees and their dependents.
All SLB noninvolved HSE events occurring
- During a vacation or days off
- As a result of self-inflicted injury such as suicide or drug overdose
- AS a result of non-occupational injury or illness
- As a result of sporting or recreational events
- Outside working hours when not in SLB controlled facilities
- Commuting to work

A work-related activity is an activity in a work environment which is or ought to be subject of management controls.
