Obstacles can prevent people from reaching their goals of improving their health and well being .

Low self-concept, lack of motivation , acceptance of current health and well being situations .

lack of time because of work and study commitment or family responsibilities .
What effects will availability or resources have ?
Lack of financial resources ,equipment or opportunities.
What effects with unachievable targets have ?
targets that are too ambitious and not Brocken down into small steps or have unrealistic timescales .
What effects will a lack of support have ?
A lack of support from family or friends , a lack of formal support , not being able to access services .
What effects will specific to the individual have ?
factors that are specific to the individual such as their age , gender ,disability, illness or addiction .
What effects will barriers to accessing identified services have ?
Geographical lotion, culture and language and psychological obstacles such as the perceived stigma of accessing a service .
Obstacles are less likely to arise as the plan will be tailored to the persons ?
needs , wishes and circumstances
If the person cantered approach is not used than the plan will not be taken into account of the persons ?
level of motivation , time issues, resources , support structures , age , gender , culture and ability, and any obstacles in any of these areas to prevent progress .
