What is periorbital cellulitis?
infectious process occurring in eyelid tissues superficial to the orbital septum
What is orbital cellulitis?
infectious process affecting muscles and fat within the orbit posterior or deep to orbital septum – no involving globe
What is epid of orbital and periorbital cellulitis?
- Children x2 likely as adults
2. Haemophilus influenza type b (Hib) vaccination decreased incidence
What is periorbital cellulitis due to?
superficial tissue injury e.g. insect bite or chalazion
What is orbital cellulitis usually due to?
underlying bacterial sinusitis
Why can peri-orbital cellulitis be worrying in children?
can be secondary to occult underlying bacterial sinusitis or due to a bacteraemic spread from primary infection – may rapidly progress to orbital cellulitis in children
What are RF for periorbital and orbital cellulitis?
- Childhood esp male
- Previous sinus infection
- Lack of Hib vaccination
What are the symptoms and signs of periorbital and orbital cellulitis?
- Recent sinus infection
- Recent eyelid injury
- Redness and swelling of eye
- Ocular pain
- Decreased vision
- Proptosis
- Eyelid oedema
- Insect bite on eyelid
- Stye or Chalazion
- Chemosis
What are possible DDx of periorbital and orbital cellulitis?
- Orbital pseudo-tumour (idiopathic orbital inflammation)
- Thyroid eye disease
- Panophtlamitis
What are investigations of periorbital and orbital cellulitis?
- Clinical Examination
- CT sinus and orbits with contrast medium
- WBC count
What would the CT show?
- inflammation of peri-orbital or orbital tissue
- sub-periosteal abscess (commonly superomedial or inferomedial)
- sinusitis
What would WBC count be?
elevated
What is the management of periorbital and orbital cellulitis?
antibiotic therapy or antifungal and if appropriate incision, drainage and culture of peri-ocular abscess
What are complications of periorbital and orbital cellulitis?
- Sub-periosteal abscess
- Cavernous sinus thrombosis
- Intracranial abscess
- Subsequent loss of vision or death
- Meningitis
What is the prognosis of peri-orbital cellulitis?
clinical improvement usually seen within 24-48 hours and full resolution
