What needs to be considered with breast discharge?
- Duct Ectasia
- Duct papilloma
- Could it be cancer?
What is the path of duct ectasia?
- Dilation of large breast ducts
2. Ducts can become blocked and lead to fluid build up
What are the key features of duct ectasia?
- Perimenopausal women (during), irregular periods
- Nipple changes: slit like retraction
- Tender lump around the areola +/- green discharge
What are Ix for duct ectasia?
often found coincidentally when doing biopsy
What is the conservative management for duct ectasia?
- Warm compress
* Abx: if redness, tenderness, fever
What is the surgical treatment for duct ectasia?
If troublesome
- Microdochectomy
- Total duct excision
What are papillomas?
benign wart like tumors
What can papillomas be made of?
- gland
- fibrous
- blood vessels
What is the patho of duct papilloma?
- Papillomas (benign wart like tumors) grow in ducts
- Can be made of gland, fibrous and blood vessels
- Occludes duct a bit
What are the features of duct papilloma?
- Clear or blood-stained discharge from a single duct
2. Rarely present as a lump
What Ix are done in a duct papilloma?
- Imaging – MMG / USS
- Galactogram – dye injected into nipple duct + X-ray (see if occlusion as dye won’t get through)
- Biopsy
What does management of duct papilloma depend on?
- Size
- Number of papilloma’s
- Associated sinister findings (FLAWS)
What is the surgical management of a duct papilloma?
Microdochectomy
