Osteomalacia Flashcards
What is osteomalacia?
softening of the bones secondary to low vitamin D levels
What is osteomalacia called when seen in children?
“rickets”
Causes of osteomalacia
vitamin D deficiency (malabsorption/ lack of sun/ diet)
chronic kidney disease
drug induced e.g. anticonvulsants
inherited: hypophosphatemic rickets
liver disease: e.g. cirrhosis
coeliac disease
Clinical features of osteomalacia
one pain
bone/muscle tenderness
fractures: especially femoral neck
proximal myopathy: may lead to a waddling gait
Describe the pattern of calcium, phosphate, vitamin D and ALP levels in osteomalacia
low vitamin D
low calcium
low phosphate (in around 30%)
raised alkaline phosphatase (in 95-100% of patients)
sign on XR
translucent bands (Looser’s zones or pseudofractures)
Management of osteomalacia
vitamin D supplementation
calcium supplementation if dietary calcium is inadequate
