irreversible inhibitor of COX-1 and 2 by covalent acetylation. decrease TXA2 and prostaglandins. increase bleed time, decrease platelets. no effect on PT, or PTT. type of NSAID. anti-platelet, anti-inflam, antipyretic, analgesic. SE: reye syndrome.
aspirin
reversible inhibitors of COX-1 and 2. antipyretic, analgesic, anti-inflam. SE: interstitial nephritis, gastric ulcers.
NSAIDs: Ibuprofen, naproxen, indomethacin (close PDA), ketorolac, diclofenac.
reversible COX-2 inhibitor. maintain gastric mucosa. spares platelet function by not inhibiting TXA2 synth. treat: RA, osteoarthritis. SE: sulfa drug, thrombosis.
celecoxib
reversible inhibits COX1 and 2 in CNS. inactivated peripherally. antipyretic, analgesic. not anti-inflamm. used in child w/viral infection. OD: hepatic necrosis.
Acetaminophen
Pyrophosphate analog. bind hydroxyapatite in bones, inhibiting osteoclasts. treat: osteoporosis, hypercalcemia, Pagets disease. SE: corrosive esophagitis, osteonecrosis of jaw.
bisphosphonates: Alendronate and other -dronates
inhibits xanthine oxidase decreasing conversion of xanthine to uric acid. increase con’c of azathioprine and 6-mp. treat chronic gout (preventative)
allopurinol
inhibit xanthine oxidase. treat chronic gout (preventative)
febuxostat
Inhibits reabsorption of uric acid in PCT. also inhibits secretion of penicillin. treat chronic gout (preventative)
Probenecid
treat pain associated with acute gout attacks.
NSAIDs: naproxen, indomethacin
GC: oral or intra-articular
Binds and stabilizes tubulin to inhibit microtubule polymerization, impairing leukocyte chemotaxis and degranulation. treat acute gout attacks.
Colchicine
Fusion protein from recombinant DNA. TNF receptor decoy. prevent activation of macrophages. treat: RA, psoriasis, ankylosis spondylitis. SE: reactivation of latent TB, infection.
Etanercept
Anti-TNF-alpha mAb. prevent activation of macrophages. treat: IBD, RA, psoriasis, ankylosis spondylitis. SE: reactivation of latent TB, infection.
