3 classifications of graft rejection
hyper acute
acute
chronic
Hyperacute rejection
occurs immediately due to pre-existing Ab to the ABO Ag or MHC Class I Ag
Preformed Ab bind complement and lead to vascular damage
Acute rejection
occurs within first few days-weeks
Due to activation of T cells in recipient to donor Ag
May be prevented by use of immunosuppressive drugs
Chronic rejection
Occurs over long period of time (even years)
Gradual loss of graft tissue structure and function
Both cellular and humoral
Graft vs. Host disease
occurs in severely immunocompromised patients
T lymphs from donor tissue recognize Ag of host as foreign and mount an immune response against them
Graft vs. host disease can also occur in immunocompetent recipients of blood from a biologically related donor because
the repent will not recognize the donor as foreign, but the donor will see the recipient as foreign
Requirement for all direct donation blood products
Blood is irradiated to prevent graft vs. host disease (kills WBC)
