Macrocytosis and Macrocytic anaemia Flashcards
What is a difficulty with measuring ferritin?
is an acute phase protein so raised in infection
What is macrocytic anaemia?
anaemia in which the red cells have a larger than normal volume
What is the units of MCV?
femtolitres
What are the causes of macrocytosis?
megaloblastic and non-megalobastic
What is a normoblast?
normal red cell precursor with a nucleus
What is the loss of the nucleus in precursors regulated by?
Hb content
what is a megaloblast?
abnormally large nucleated red cell precursor with an immature nucleus
what are megaloblastic anaemias characterised by?
defects in DNA synthesis and nuclear maturation with relative preservation of RNA and haemoglobin syhnthesis
What are the causes of megaloblastic anaemia?
B12 deficiency; folate deficiency; drugs; inhierited abnormalities
Why does B12 and folate deficiency cause megaloblastic anaemia?
essential co-factors for nuclear maturation
What is the folate cycle important for?
nucleoside synthesis- uridine–thymidine conversion
what is pernicious anaemia?
autoimmune condition with resulting destruction of gastric parital cells
How long do body stores of B12 last?
2-4years
What are the causes of folate deficiency?
inadequate intake; malabsorption; excess utilisation- haemolysis; exfoliating dermatitis; pregnanyc; maligannt; anticonvulsants
What are the signs of B12 and folate deficiency?
weight loss; diarrhoea; infertility; sore tongue; jaundice; developmental
