Technique of Median Episiotomy
Incision is done in midline of perineum.
Structures Divided in Median Episiotomy
- Vaginal mucosa & fourchette.
- Perineal skin & fascia.
- Perineal body.
Disadvantages of Median Episiotomy
May extend to involve anal sphincter.
Advantages of Median Episiotomy
- Easy.
- Less bleeding.
- Repair is easy & anatomical (less discomfort, dyspareunia).
Technique of Mediolateral Episiotomy
- Incision starts at fourchette then extends towards ischial tuberosity.
Structures Divided in Mediolateral Episiotomy
- Vaginal mucosa & fourchette.
- Perineal skin & fascia.
- Superficial & deep transverse perineal muscles.
- Bulbocavernosus muscle.
- Pubovaginalis muscle
Advantages of Mediolateral Episiotomy
Avoid extension to anal sphincter.
Disadvantages of Mediolateral Episiotomy
- More difficult.
- More bleeding.
- Repair is difficult & not anatomical (more painful scar).
Technique of J-Shaped Episiolotomy
Incision starts as midline episiotomy to within 2 cm from anal sphincter then extends laterally
