Etiology
look at the triggers
idiopathic
linked diet, stress, smoking, lactose intolerance
Patho
malabsorption of fermentable CHO and polyps (alcohol sugars) - when processed by the gut flora = gas (flatulence)
altered CNS regulation of GI motor and sensory function
molecular signalling defect for serotonin
What is serotonin
neurotransmitter produced in the gut mucosa
what is serotonins role in IBS
used for motility
Manifestations
abdominal pain abdominal discomfort diarrhea or constipation flatulence nausea mucoid stool
what is mucoid stool
when the gut is angry it secrets more mucous (excreted in the stool)
Diagnosis
exclude other disease
Presentation
Labs: BCB, CRP
Scopes: endoscopy, colonoscopy, sigmoidoscopy
Treatment
based on severity and type
avoid offending foods
decrease stress
drugs to assist with manifestations: laxatives, analgesics, antibiotics with cautions (don’t want to eliminate good gut flora)
