Herniation Flashcards
what is a hernia
organ protruding through a retaining surface
where do they usually occur
in the abdominal cavity
Patho
weakened supporting structures- congenital or acquired
increased intra abdominal pressure (pregnancy/obesity)
What are the two kinds of hernias
Hiatal
Inguinal
Where does a hiatal hernia occur
in the hiatus (hole in diaphragm for esophagus)
how does the stomach enter
when the hole weakens the stomach could enter the thoracic cavity
what are the two types of hiatal hernia
Sliding
Paraesophageal
what is another term for paraesophageal hernia
rolling hernia
Describe the sliding hernia
most common 95% GEJ and upper stomach enter TC some asymptomatic chest pain heartburn gastric reflux
Describe the rolling hernia
non upper part of stomach enters TC GEJ stays below the diaphragm chest pain dyspnea fullness after means no reflux as GEJ stays below
Treatment
lifestyle modifications (diet) elevate HOB dt reflux drugs - antacids, H2RA, PPI surgery - fundoplication sutures used to wrap fundus around GEJ
What is occurring with an iguana hernia
abdominal organs are protruding via inguinal ring
what forms the hernial sac
peritoneum
what does this sac usually contain
intestines and omentum
Treatment
surgical correction to reduce hernia (push back) and tighten up inguinal ring
