What is Cirrhosis
end stage liver disease
what is occurring in cirrhosis
the liver is undergoing fibrosis = scar tissue
which makes it a nodular liver
what is the primary problem with cirrhosis
decrease liver function and portal HTN
what is the main cause of cirrhosis
alcohol abuse
another leading cause
hepatitis (HepC)
Etiology
drugs (methotrexate: prevents cell division)
Biliary disease
metabolic disorders
cryptogenic
what is an example of a metabolic disorder
hemochromatosis
when iron does metabolize and deposits on the liver
Manifestations
can very
common: anorexia, weakness, weight loss
hepatomegaly
jaundice
what are the complications that arise
portal HTN which may lead to ascites
Varies which may lead to GI bleeds
Splenomegaly (fluid shift dt HTN and enlarges the spleen)
Patho
Hepatocytes are destroyed. the cells regenerate and fibrous scar tissue forms
how do the complications arise
vessel constriction dt fibrous tissues impede perfusion - fortal HTN occurs
because of the HTN You have an increase in hydrostatic pressure - fluid shift occurs = ascites
duct constriction dt fibrous scar tissues impedes bile flow = bile stasis
decrease metabolic clearance = liver failure
Treatment
decrease workload of the liver
- diet small meals, increase cals, decrease fat
- NO alcohol or hepatotoxic drugs
- treat complications as they develop
