What is intussusception?
Invagination of one portion of the bowel into the lumen into adjacent bowel.
Most common in ileo-caecal region.
Who is intussusception more common in?
6-18 months
More common in boys
What associated conditions can you expect with intussusception?
Concurrent viral illness
Meckel’s diverticulum
How can a recent viral illness increase risk of itussusception?
Hypertrophy of peyer’s patches in small intestine can predispose
How to diagnose intussusception?
- Severe colicky abdominal pain
- Red current jelly stool
- RUQ sausage shaped mass
- Drawing legs ups to abdomen
What is seen on US?
Target like mass
What imaging modality is used to intussusception?
Ultrasound
How to manage intussuception?
ABCDE Fluids, NBM + NG tube aspiration
1st line in stable child : Rectal air insufflation or reduction via contrast
What to do to manage intussusception if 1st line fails?
Surgery:
Immediate laparotomy
What are the indications of immediate laparatomy (usually surgical management if 2nd line)
Unstable
medical tx failed
Peritonitis
Perforation
What to investigate ig a baby has recurrent intussuception?
Lead point (meckel’s diverticilum, polyps, appendix)
