Signs and symptoms
HAND FOOT MOUTH
Painful + itchy - vesicular lesions
hand foot mouth + tongue + bum
fever + sore throat + spots in mouth become ulcers
Management
HAND FOOT MOUTH
Supportive to clear in 10 days
What organism causes roseola infatum?
ROSEOLA INFANTUM / HHV6
HHV6
What aged children does it affect
ROSEOLA INFANTUM / HHV6
Most kids have had infection by age 3 as is highly contagious
Signs and symptoms
ROSEOLA INFANTUM / HHV6
High fever + malaise
Small pink spots = macular rash appearing as fever wanes
Rash spread?
ROSEOLA INFANTUM / HHV6
Starts on neck / body and spread to arms
+ Nagayama spots : uvula and soft palate spots
Differentials for Roseola infatum?
ROSEOLA INFANTUM / HHV6
Measles and rubella so need to get serology
How to investigate children over 18 months
HIV
antibody detection via ELISA
How to investigate in children under 18 months?
HIV
still have transplantal anti-HIV igG from mum
PCR of virus, measured at birth, 6 weeks, 12 weeks, 18 months
Management at birth?
HIV
Urgent cord clamping and baby washed
ZIDOVUDINE monotherapy for 2-4 weeks
or
PEP combination if high risk
Mum does NOT breastfeed
