pneumonia, disseminated infx, nasty mucosal ulcerations, lymphadenopathy

azoles or TMP-SMX et al

Which bug? Sx? Where?

A

Histoplasma capsulatum (intramacrophage). Ohio, bird or bat poop. Most asymptomatic; can present as pneumonia with calcifications or disseminate like TB.

Tx: amphotericin B, itraconazole

A

Blastomyces (broad yeast). Southwest. Nasty skin lesions, pneumonia, may disseminate.

Amphotericin b, azoles.
