What type of drug is fentanyl?
A fast acting synthetic narcotic analgesia
What is the action of fentanyl?
Acts on the CNS by binding to opioid receptors
What are the two indications for fentanyl?
- Moderate to severe pain
2. Safe analgesic to those with anaphylactic reactions to morphine
What are the 5 adverse effects of fentanyl?
- CNS depression
- Respiratory depression
- Nausea/vomiting
- Occasionally - bradycardia
- Rigidity of the diaphragm, intercostals and jaw (rare)
What are the 4 contraindications for fentanyl?
- KSAR
- Bleeding or bilateral occluded nostrils
- ALOC
- <1 year
What is the dose and repeat dose for fentanyl for 1-5 years (<20kg)
25 mcg repeat 25mcg every 10 mins as required
What is the dose of fentanyl for 5-12 years (20-40kg)
50mcg repeat 50mcg every 10 mins as required
Fentanyl dose for small adult (40-70kg)
100mcg repeat 50mcg as required
Dose of fentanyl for 12+ to adult years ( over 70kg)
200mcg repeat 50mcg as required
What are the 5 special notes with IN fentanyl?
- Alternate nostrils per spray
- Doses of morph should be given at 1/2 dose following fentanyl
- Fentanyl should not be administered after an APS max do 20mg morphine
- Antiemetics should only be administered if the pt has nausea or vomiting prior or post fentanyl
- Intranasal fentanyl should NEVER be administered IV
