What are the goals of patient centred care?
Safe effective patient care by:
- treating all patients, carers and family’s with dignity and respect
- encouraging shared decision making
- communicating information about condition/treatment
- obtaining consent and considering pts wishes and values in all decisions
What is the stepwise approach to pain management?
- Non pharmacological options (position, reassurance, cooling of burns, temp control)
- Inhalation/enteral medication (methoxy, GTN, provide advice on and assistance with OTC medications with caution)
- Parenteral medication (morphine, fentanyl)
What are the 3 goals of airway management in order?
- Oxygenation
- Ventilation
- Airway protection
What is the adult ratio (>12) of compressions:ventilations?
30:2
What does COACHED stand for?
Compressions continue Oxygen away All else clear Charge Hands off Evaluate Defib or dump
What are the 4 reversible H’s and Ts?
Hypoxia
Hypovolameia
Hyper/hypokalaemia
Hyper/hypothermia
Tension pneumo
Tamponade
Toxins
Thrombus
In an adult SHOCKABLE cardiac arrest, when is IV adrenaline given?
After 2nd shock and then in every second loop
In an adult NON-SHOCKABLE cardiac arrest, when is adrenaline given?
Immediately and then in every second loop
What are the 4 respiratory goals in the ROSC patient?
- Consider advanced airway if deceased LOC
- Maintain SpO2 94-98%
- Ventilation 8-10/min titrated to SpO2 and etCO2 (do not hyperventilate)
- EtCO2 between 35-40mmHg
In regards to ROSC pts what are your goals with temperature and blood sugar
- Maintain normothermia
- No active rewarding unless profoundly hypothermic (<32)
- Do not allow pt to shiver
- Avoid hypoglycaemia but do not over correct it
What are the cardiac/perfusion goals in a ROSC patient?
12 lead
Maintain cerebral perfusion. Aim for BP >100mmhg by any means (posture/ fluids)
