when can Eisenmenger syndrome develop
after 1-2 yrs large shunts
in adulthood small shunts
Eisenmenger syndrome - pregnancy
develops more quickly in pregnancy
pathphysiolgy
overtimer pulmonary htn and R sided overload
pulmonary pressure eventually exceeds systemic –> R to L shunt
deoxygentated blood bypassess lungs entering systemic circulation –> cyanosis
how does bone marrow respond to cyanosis
produced more RBC and Hb
this leads to polycythemia –> blood more viscous and pt at risk of stroke
pulmonary htn - OE
R ventricular heave
loud S2
raised JVP
peripheral oedema
chronic hypoxia - OE
cyanosis
clubbing
dyspnoea
plethoric complexion due to polycythemia
prognosis
reduce life expectanct 20yrs
main causes death - heart faillure, infection, VTE, haemorrhafe
up to 50% mortality in pregnancy
definiitve Mx
heart lung transpplant
medical Mx
oxygen
pulm htn - sildenagil
venesection (polycythaemia)
anticoagulation
