bronchiolitis Flashcards
what
inflammation and infection bronchioles
most common cause
RSV
features
coryzal symptoms signs resp distress dyspnoea tachypnoea apnoea wheeze crackles
signs of resp distress
tachypnoea use accessory muscles recessions nasal flaring head bobbing tracheal tug cyanosis wheeze grunting stridor
reasons for hospital admission
<3mo preexisting condition - premature, Down's, CF dehydrated RR > 70 O2 sats < 92% resp distress apnoea
supportive management
ensuring intake
nasal suction
O2 if <92%
ventilatory support
High-flow humidified oxygen via tight nasal cannula
delivers air and oxygen continuously with added pressure –> oxygenates lungs and keeps them from collapsing
adds PEEP to maintain airway at end of expiration
CPAP
sealed nasal cannula
delivers oxygen in similar way to high-flow humidified oxygen but much higher and controlled pressures
Palivizumab
monoclonal antibody against RSV
given as monthly injection as RSV prevention to babies at high risk
provides passive protection (so not true vaccine)
