Furosemide and ethacrynic acid

Furosemide:
A
sulfonamide loop diuretic
inhibit cotransport system (Na, K, 2Cl) of thick ascending.
Abolishes hypertonicity of medulla, preventing concentration of urine.
Stimulates PGE release (vasodilatory effect on afferent)
Increases Ca2+ excretion
“Loop loses Ca2+”

A
Edematous states (CHF, cirrhosis, nephrotic, pulm edema)
HTN
Hypercalcemia

Ototoxicity Hypokalemia Dehydration Allergy (sulfa) Nephritis (interstitial) Gout

Ethacrynic acid
A
Phenoxyacetic acid derivative (not sulfonamide)
Essentially same action as furosemide

Diuresis in pts with sulfa allergy

Similar to furosemide
Can cause hyperuricemia,
NEVER use to treat gout
