Hot Axle Box

A

Low grease in wheel bearings causes heat. Test if hot with wax/water/may glow.

Can’t start from depot
Can’t start in service

If HABD activated & checked in 10 mins:
If hot= 10mph to clear line & 5mph over points
If not hot= Proceed as normal & pass HABD in 50 miles. If activated again, do not check but 20mph as precaution

If HABD activated & not checked in 10 mins:
20mph as precaution

If unsafe to move, RST inspection. Do not move until authorised.

A

Isolate air suspension reservoir & deflate all bags on affected vehicle.

Can start in service at max.45mph if pax in affected vehicle

In service, inform signaller immediately, may remain in service at max.45mph if pax in affected vehicle. Line speed if pax moved.

Complete RT3185 & AWS to be isolated due to not cancelling warnings

- Competent person (route and traction) in leading cab whilst train driven from other than leading cab (max.20mph or max. 5mph if relying on parking brake in leading cab)

A

- Competent person (PTS trained)- line speed
- Driving alone- 60mph

In service, inform signaller- out of service and then ECS

If both AWS and DVD defective, competent person to be provided before train moves.

In service, inform signaller. May continue current journey with competent person in rear unit. Move pax to front unit (if not possible, out of service & ECS to repair location).

If travelling over areas not TCB, competent person to travel in rear unit in case of division to contact signaller and lay emergency protection. If train divides with EBS in leading cab, TW4 will not apply brakes on driven unit.

In service, report to signaller. May continue current journey if competent person in cab with operative GSMR (guard to remain in cab when not carrying out dispatch & apply emergency brake on hearing emergency call, then inform driver).

Can start in service if only one headlight defective and other can be used or complete headlight failure and portable headlight available (max. 75mph).

Can’t start in service if complete headlight failure and no portable headlight available.

In service, inform signaller:
If one headlight failed- remain in service using other
If complete headlight failure, can remain in service with portable headlight (max. 75mph)
If complete headlight failure and no portable, out of service and then max. 20mph with bardic lamp and must continuously sound horn

Tail light

A

Can’t start from maintenance depot

Can start in service if one or both tail lights fail and bardic lamp available to show single red aspect at rear

In service, inform Control. May remain in service with red aspect on bardic at rear

Minimum requirement is one red aspect at rear

Can start in service if clear view and inspected by RST

In service, inform signaller- remain in service if clear view. If no clear view, out of service and safe reduced speed. Must continuously use warning horn.

In service, inform control- may remain in service but must end day at maintenance depot

New vehicle may be attached to bury fault

In service, report to signaller - may remain in service and pax removed from affected vehicle or if guard can access all vehicles. If guard cannot access all vehicles, out of service & ECS to repair location.

Guard must have method of applying emergency brake- if EBS operated, follow EBS instructions.

A

Can start in service if affected vehicle not used

In service, report to Control- may remain in service and pax moved from affected vehicle (if not possible, guard to make face-to-face announcements).

If hustle alarm and PA inoperative, out of service at first opportunity.

Can start in service if driver believes they will have no issue stopping.

In service, report to Control. If driver believes they will have no issue stopping, may remain in service. If driver may have difficulty stopping, out of service at next opportunity (speed at driver’s discretion)

Can’t start in service

In service, inform signaller immediately. Out of service at next opportunity- reduce speed to comply with all speed restrictions.

TCA

A

Can’t start from depot

Can start in service if one TCA working on first two vehicles & one TCA working in last two vehicles (1 or 2 car with at least 1 TCA will always meet criteria).

In service, inform signaller immediately if no operative TCA. Out of service at next opportunity. Approach automatic level crossings at caution.

Control & Network Rail may agree for train with one TCA to complete current journey only.

TIS

A

Can’t start from depot

Can only start as ECS to repair location

In service, inform signaller. Out of service at next opportunity then ECS after driver & conductor physically check doors are closed

Driver must reinstate TIS when changing ends, stabling or handing over to another driver.

Warning Horn

A

Can’t start from depot if both tones defective

Can start in service if at least one tone working. Not if both defective

In service, report to Control- may remain in service if one tone defective and continue at line speed. If both tones defective, inform signaller immediately, out of service and max. 20mph.

Wheel flats, locked wheels, shifted tyres or dragging brakes

A

Can’t start from depot with any.

Can’t start in service with any.

In service, inform signaller immediately:
- Examine brakes, tyres, wheels for damage/overheating
- If brakes cannot be fully released, must be isolated (if not fully released, max. 10mph and 5mph over points)
- Must check wheels rotate freely before proceeding (rotation test)
- If wheels can be freed & no damage= proceed normally
- If wheels can be freed but 60-100mm flats = report to signaller, do not move until instructed- when authorised, max. 20mph unless RST advises otherwise
- If wheels can be freed but 100mm or longer flats/flange formed/suspect shifted tyre= report to signaller, do not move until instructed and examined by RST and authorised by signaller.
- If wheels cannot be freed, report to signaller, do not move until examined by RST and authorised by signaller.
