15
Q

What is different about the 1st rib?

A

It has a groove for the subclavian vein and a groove for the subclavian artery.

It has scalene tubercle.

It only has one facet on the head.

18
Q

What body structure is also called the angle of louis?

A

The sternal angle

19
Q

What nerve would you hit if you place too low in a thoracentesis?

A

The collateral branch of the intercostal nerve

20
Q

What type of joints are the costochondral joints?

A

Synchondrosis

21
Q

What is different about the 2nd rib that makes it an atypical rib?

A

It has the tuberosity for the serratus anterior

22
Q

To which joint is the radiate ligament associated?

A

The costovertebral joints

23
Q

Where are chest tubes typically placed?

A

Between the 5th and 6th ribs

24
Q

What about the 10-12th ribs make them atypical?

A

They all only have one articulating facet

They don’t have tubercles or necks

25
Q

What structures make up the inferior thoracic aperature?

A

12th thoracic vertebra

11th / 12th ribs

Xiphisternal joint

26
Q

What is the order of the grooves for the subclavian vein and artery on the first rib?

A

the groove for the vein is more anterior/superior than the artery

basically, veins are pretty much always in front of the artery

27
Q

CN: Rib Fractures

A

most commonly the middle ribs at the weakest point, the costal angle

can damage internal organs, produce severe pain when breathing, coughing, laughing, sneezing, etc.

28
Q

CN: Supernumerary ribs

A

extra ribs, either cervical or lumbar

typically asympotmatic unless cervical is compressing brachial plexus or subclavian A. (thoracic outlet syndrome)

lumbar ribs can be confusing on xray

29
Q

what type of joint is the manubriosernal?

A

symphysis between sternal angle and 2nd costal notch

30
Q

What type of joint is the xiphisternal joint?

A

synchondrosis between thorax and 7th costal notch

31
Q

What type of sternocostal joint is between rib 1?

What about between ribs 2-7?

What ligament is involved?

A

syncohondrosis

synovial planar

all held together by anterior and posterior radiate sternocostal L.

32
Q

What type of joint is the costovertebral joint?

What are the ligaments involved?

A

synovial planar between the ribs and the thoracic vertebral

radiate L.

intraarticular L. (to IV disc)

articular capsule

33
Q

How to name ribs/connections with thoracic vertebra?

A

example: rib five articulate with T5 wearing the “hat” of T4

34
Q

What type of joint is the costotransverse joint?

What two ligaments are involved?

A

synovial planar between rib and TP of same number

lateral costotransverse L. (from tubercle of rib to TP of same number)

superior costotransverse L. (neck of rib to TP above)

35
Q

Bucket handle movement is the motion of which part of the rib?

Pump handle movement is the motion of which part of the rib?

A

lateral portion of the rib

sternum/sternal end of ribs

all to increase volume of thorax needed for respiration

36
Q

What type of joint is the costochondral joint?

A

synchondrosis, between ribs and costal cartilages

37
Q

What type of joint is the interchondral joint between 6-9 ribs? 9-10 ribs?

A

planar synovial

fibrous joint

held by interchondral L.

38
Q

External intercostal muscles function to?

Internal intercostal muscles function to?

A

elevate ribs

depress ribs

39
Q

The NV bundle of the ribs is found

A

between the internal intercostal m. and the innermost intercostal m. at the inferior border of the superior rib sending collaterals to the superior border of the inferior rib, thus aim for dead center when sticking something between the ribs

(when looking into the chest wall, if you see the NV bundle, no innermosts are present, but once they dive behind a structure if it’s only passing from on rib to the next, it’s an innermost)

40
Q

CN: Thoracentesis

A

insertion of needle through intercostal muscles between ribs to obtain fluid or small sample from pleural cavity

41
Q

CN: Insertion of vhest tube

A

aim between ribs 5-6

42
Q

CN: thoroscopy

A

inserting a thoroscope into pleural cavity though small incision

43
Q

Transverse thoracic muscles are where and do what?

subcostal muscles are found where?

A

posterior surface of anterior chest wall near sternum, depress ribs

interior posterior thoracic wall, traveling from rib to rib, jumping over one in the middle (that’s how you know it isn’t an innermost intercostal m.)

44
Q

Rib dislocation vs. Rib separation

A

D: sternocostal joint

S: costochondral joint
