Cirrhosis Flashcards
What are the 4 portal-caval anastemoses?
- oesophageal and gastric venous plexus
- unbilical vein from the left portal vein
- retroperitoneal collateral vessels
- rectal (haemorrhoidal) venous plexuses
What is the normal portal vein pressure?
5-8 mm Hgq
What is portal hypertension?
Portal vein pressure above 8 mmHg
What are the causes of portal hypertension?
- prehepatic:
- blockage of portal vein before the liver - intrahepatic:
- distortion of the liver architecture
Signs of compensated cirrhosis?
spider naevi palmar erythema clubbing gynaecomastia hepatomegaly pleenomegaly or None
Signs of decompensated liver disease?
Jaundice
ascites
encephalopathy
bruising
Treatment of decompensated liver cirrhosis?
- high nutritinal intake
- small frequent eals - prevent gluconeaogenesis and muscle catabolism (prevent fasting)
- supplement Vitamin B and Thiamine (if alcoholic)
- supplement Vitamin D
What is the cause of ascites in cirrhosis?
Failure to perfuse fibrosed liver causes the liver to produce vasodilators.
Splanchnic arterioles are vasodiated.
Renal vasoconstriction - Na and water retention.
Ascites treatment?
- Spironolactone (diuretics)
- low Na diet
- TIPSS (shunt between portal and hepatic vein)
- transplantation
Sign of liver Encephalopathy?
Asterixis (liver flap)
Ammonia is involved in pathology
Treatment of oesopageal varical bleeding?
- endoscopic banding
- balloon tamponade (gastric balloon)
Patients with liver disease coagulation dysregulation causes them to…
be both prone to bleeding and excess coagulation
