Moderate:        - 4mg PO, 10mg IV/IM
Severe / Anaphylaxis:            - 10mg IV/IM

Mild: 6-11years - 2mg PO
≥12 years - 4mg PO

Moderate: <1 year 0.25mg/kg IV/IM
1 - 5 years 2.5mg IM/IV
6 - 11 years 2mg PO, 5mg IV/IM
≥12 years 4mg PO, 10mg IV/IM

Severe/: <1 year 0.25mg/kg IV/IM
Anaphylaxis 1 - 5 years 2.5mg IM/IV
6 - 11 years 5mg IV/IM
≥12 years 10mg IV/IM
