Ch 49 disorders of parathyroid Flashcards
what does parathyroid hormone (PTH) regulate (2)
calcium
phosphorus
3 causes hyperparathyroidism
primary: from benign tumor
secondary: from conditions affecting vitamin D (ex: end stage renal disease)
tertiary: loss of negative feedback loop
S+S hyperparathyroidism (3)
- decreased bone density (osteoporosis)
- kidney stones
- constipation
treatment options for hyperparathyroidism (4)
- surgery
- bisphosphonates (for osteoporisis)
- calcimimetics
- diuretics
important nursing consideration for administration of bisphosophonates
sit up for 30 min after taking
how should you treat acute hypercalcemia (2)
- 0.9% NaCl
- furosemide
how would decreased PTH levels affect calcium and phosphorus levels
decrease calcium
increased phosphorus
how would increased PTH levels affect calcium and phosphorus
increase calcium
decrease phosphorus
why is mobility and exercise important for pt with hyperparathyroidism
they already have decreased bone density because of too much calcium in bones
immobility worsens bone loss
most common cause hypoparathyroidism
surgery removing thyroid
S+S hypocalcemia that can accompany hypoparathyroidism
- laryngeal stridor
- chvostek sign
- trousseau sign
treatment hypocalcemia (2)
- rebreathe into paper bag
- give calcium gluconate IVPB
treatment hypoparathyroidism (3) lifelong!
- correct hypocalcemia
- correct hypomagnesemia
- correct vitamin d deficiency
