Controlled substances
any drug listed on one of the “schedules”of the controlled substance act
drug polymorphism
variation in response to a drug because of a patient’s age, gender, size, and or body composition.
Over the counter drugs (OTC), non-perscription drugs
drugs available to consumers without a prescription.
adverse drug reactions
unexpected, unintended, undesired, or excessive responses to medications given at therapeutic dosages
idiosyncratic reaction
any abnormal and unexpected response to a medication, other than an allergic reaction, that is peculiar to an individual patient.
medication reconciliation
assess and update medication history with communication to health care providers. Done at admission, status change, transfer, and discharge.

any preventable event that MAY cause or lead to inappropriate medication use or patient harm.
If medication error occurs
- Assess the patient’s condition
- Assess for effects
- medication error form and inform physician, pharmacy, and supervisor
- monitor pt

- route
- time (frequency)
- Signature of prescriber
standard order
apply indefinitely until d/c or change in status.
