cardiac output is insufficient to meet body demands

- loss of myocardium (IHD, cardiomyopathy)
- Pressure overload (aortic stenosis, hypertension)
- volume overload (aortic regurgitation, shunts)

- hypertrophy

- most commonly due to left heart failure
