Cause of the problem?
75% of the nephrons are lost.
Caused by an abnormality or disfunction of the kideny
Clinical signs
Depends on the staging
Main;
- PU/PD, vomiting, weight loss, dehydration, uremia
Oral ulceration, anaemia, osteoporosis, convulsions, death
Diagnosis
Based on clinical signs, history.
o Moderate/severe non regenerative anaemia
o Serum creatinine and Pi are increases, metabolic acidosis, hypertension
At the clinic; symmetric dimethylarginine (SDMA) as a surrogate marker of glomerular filtration rate,
Rubber jaw?
Caused by secondary renal hyperparathyroidism.
Than lead to CDK; Increased ca mobilisation from the bones, realinsing it into blood –> excreted by kidney
=> renal osteodystrophy of the mandible
Nutritional management;
Decrease in proteins. (To prevent uremia)
Ca: 14 – 18% (2 – 2.2 g/kg W)
Fe: 25 – 28% (3.3 – 3.4 g/kg W)
Inc with fats and CH (ME) to make up for the reduced proteins
Dec; P, Na
Inc; K, antiox, vitB, fibers (promote gut health)
Inc CH –> helps to dec ammoina absopbed.
Omega 3+6
What is the IRIS-KIDNEY diagnosis?
Staging of the degree of the disease. (1-5)
International Renal Interest Society =IRIS
At which stage of CKD should the prescription diet feeding be starting?
Stage 2 (increased creatintine), SDMA
What physical form of prescription diet should be given?
Moist/canned food
