IgG antibofy - mediated cell destruction

hemolytic anemia , trombocytopenia, neutropenia
type 3
delayed IgG - drug immune complex deposition
1-2 weeks after
low completment C3-C4 and high ESR
serum sickness - fever, rash, arthralgie glomerulonephrite
vasulite (with lower extremities)
type 4
delayed T cells
more than 48 h / days weeks after exposure
BUT when you get a re-ex^psure = less than 24h
DRESS: eosino^philes ++
SJS TEN - severe exfoliative derhamatose
contact dermatitis
Dose per kg of epi recomended
0.01 mg per kg
dose for adult of epi
0.5 mg IM
dose for a child of epi
0.3
adjunct to EPI
glucagon 1-5mg IV over 5 min
salbutamol for bronchospasm
antihistamine
gluco - no evidence that it helps
when to give glucagon
if on BB and poor response to epi
when to observe for a biphasic reaction
if hx of biphasic reaction
If severe sx refractory hypotTA, laryngeal edema, respiratory compromis
Delayed or suboptimal treatment
discharge mngm plan (4)
1) teach + printed explanation
2) prescribe epi
3) medicalert bracelet
4) referral for allergy consult if unknown trigger
teaching when to call 911
1 inject
2 call 911
3 inject prn
