A
- Conditions where pulp and periodontium diseased simultaneously
- Disease may arise from one or both sites

- Root canal infection leaks through apical foramen
- Caries, restorative procedures, extrusion of toxic medicaments/root canal materials and trauma common aetiologies
- Disruption of apical lamina dura or widening of PDL space radiologically

- Clinical signs of pulpitis may or may not be present
Clinical signs of pulpitis-spontaneous pain, thermal sensitivity, tenderness to percussion

- Through PDL space and exiting at bottom of sulcus
- Perforation of cortical bone close to apex=>may develop sinus tract

A
- Occurence of sudden pain
- Marginal inflammation
- Increased probing depth
- Suppuration
- Increased mobility
- Loss of fibrous attachment
