26 Pulmonary Vascular disease Flashcards
PE Long term consequences
Obstruction of proximal and distal pulmonary arteries
PE testing
VQ scan- ventilation and perfusion don’t line up
Pulmonary hypertension classification
- Pulmonary arteral hyppertension- precapillary
- Pulmonary venous htn- post capillary- LHF *Most
- Respiratory PHTN- i.e. fibrosis
- Thromboembolic disorders
- Other disorders
PAH 1
Idiopathic, Familial (BMPR2), Scleroderma/CollagenVD
Mean pulmonary artery pressure >25 mmHg at rest
Normal PCWP
Histo-
Arterial thickening
Plexiform lesions
PH2
LHF- PAP>25, CWP elevated
PAH1 pathogenesis
Increased endothlin pathway- constriction and smc proliferation
Decreased NO pathway
Decreased prostacyclin
Treat with NO pathway treatments like sildenafil
Goodpastures syndrome
Anti Basement membrane of capillaries
Granulomatosis with polyangiitis
c-ANCA associatted granulomatosis vasculitis
Necrotizing granulomas hemorrhage
Treatent
Prednisone
Idiopathic hemosidarosis
Increased hemosideran laden macrophages on bronchoscopy lavage.
