16: Drugs for PAD and DVT Flashcards
PAD: Peripheral artery disease
narrowed arteries -> reduced blood flow to limbs -> claudication (leg pain with walking)
sx of PAD
- painful cramps in hips, thighs or calves after certain activities
- numbness, weakness, weak pulses, coldness
- skin changes: shiny, hair loss, slow toenail growth
- sores on feet/legs that dont heal
- ED in men
most common cause of PAD
atherosclerosis
RFs of PAD
smoking**, DM, obesity, HTN, HLD, age 50+, FHx PAD/CAD/CVA, high homocysteine
critical limb ischemia
sores that dont heal -> tissue gangrene -> may require amputation of affected limb
prevention of PAD
- stop smoking
- exercise regularly
- control comorbidities
- avoid vasoconstricting drugs
ankle-brachial index
ratio of BP in ankle:arm
normal range for ankle-brachial index
1 - 1.4
the one drug used for PAD
cilostazol
three groups of drugs that interact with warfarin
- drugs that increase warfarin effects: inhibit degradation / decrease clotting factor synthesis
- drugs that promote bleeding
- drugs that decrease warfarin effects: oral contraceptives, vit K, etc.
two blood tests to monitor heparin levels
aPTT, anti-Xa
blood test to monitor warfarin level
INR
blood test to monitor DOAC factor Xa inhibitors
anti-Xa
blood test to measure DOAC direct thrombin inhbitor levels
TT (thrombin time)
