REPORTING:

  1. In a critical or emergency situation in which a serious threat or injury occurs or is likely, or, if need of immediate assistance, CONTACT ___ IMMEDIATELY AND BUILDING SECURITY.
  2. In all other situations of workplace violence, any employee who believes that they have been subjected to workplace violence, shall report the incident to their supervisor ___, and in no event later than ___ days after the incident occurred. In any situation that the employee’s immediate supervisor was involved in the incident of workplace violence, the employee shall report to the ___, ___ or ___ head.

RESPONSIBILITIES:

  1. Upon report of an incident of workplace violence, the Manager, Commissioner or Department head shall, in conjunction with __ ___, conduct an ___ into the matter. The investigation shall be conducted in a f__, c__, i__ and s__ manner. Their responsibilities will include:

DISCIPLINE:
A. City employees who are found to violate this policy will be subject to ___ action, up to and including termination of employment. Persons involved may be also subject to c__ and c__ liability.
