Stage One
(Reddening of the skin). Skin is unbroken and wound is superficial. Like a sunburn.
Tx: turning or alleviating pressure in some form or avoiding more exposure to the cause of injury.
Stage Two
(The blister is either broken or unbroken). A partial layer of skin is now injured. (Must cover, protect & clean area). Quick attention to Stage One decubitus is very important.
Stage Three
(The wound extends through the layers of the skin). It’s a primary site for a serious infection to occur.
Tx: Treat as Stage Two. Medical care is necessary to promote healing & prevent infection.
Stage Four
(Extends through the skin & involves underlying muscle, tendons & bone). Depth, not diameter, is important. Very serious & may be life-threatening.
Tx: Must have medical care. Surgical removal of the necrotic or decayed tissue is often used in wounds of large diameter.
