what is a home-birth?
A birth has been pre-organised to occur at home
suitable for low risk pregnancies
what is required for a home-birth?
2 APRAH registered health practitioners are required at all homebirths
one must be a midwife
the second must be educated to provide maternal and newborn care
treating team capable of recognising and stabilising unwell women and newborn
what is an inter-hospital transport?
when a women transport from one hospital to another
midwife escort may accompany the woman unless low risk or not feeble
what is a birth before arrival?
an unexpected birth that occurs wither in an inappropriate location or without an experienced birthing practitioner or both
what is the incidence of births before arrival in Victoria?
BBA 0.4% of all births
most BBAs -uncomplcated
what is the pre-hospital managment for BBA
rest and reassurance
case dependant
what are the pinks?
between 24 and 48 hours post delivery
elvated mood, excitement, difficulty sleeping
what are the blues?
from delivery to 2 weeks
up to 80% of mums
fatigue, short temper, depressed, tearful, normally mild, thought to be sudden drop in progesterone and oesgtogern
what is post natal depression?
depression that starts within 12 months usually in first 6 weeks
10-15% of mums
most recover spontaneously 3-6 months
what are the symptoms of PND?
early morning waking poor appetite low energy and libido loss of enjoyment lack of intrest impaired concerntration tearfulness feelings of guilt and failure anxiet self harm and suicidal ideation
what are the risk factors for PND?
history of psychiatric ilness difficult labour isolation relationship difficulties bereavement-loss
what is the managment for PND?
rest and reassurance history PIPER ensure safety if neonate manage as per normal depressive episodes transport
what is puerperal postnatal psychosis?
very severe postpartum psychiatric disorder
sudden onset- rapidly changing presentation
what are the risk factors for puerperal postnatal psychosis?
history of postnatal psychosis
severe PND
family history
what are the symptoms for puerperal postnatal psychosis?
restless agitation insomnia perplexity/confusion fear/suspicion delusions/hallucinatons failure to eat/drink tights of self-harm depressive symptoms
