What is the cause of UC?
Unknown
- Indicative of genetic susceptibility
High conservation with twins
What is UC?
Relapsing remitting inflammatory disorder of the colonic mucosa that starts at the rectum and moves up the way.
What type of mucosa is it in UC?
Hyperaemic/haemorrhagic granular mucosa w/ pseudopolyps
Ulceration
What are the symptoms of UC?
Diarrhoea with blood and mucus
Cramping
Discomfort
Systemic features during attacks
- Fever
- Malaise
- Anorexia and weight loss
- Tachycardia
- Tender distended abdomen
Extraintestinal signs?
Clubbing
Apthous ulcers
Pyoderma gangrenosum
Erythema nodosum
PSC
Tests?
Stool MC&S (C.diff toxin)
AxR
Sigmoidoscopy w/ biopsy
Barium enema
Colonoscopy
What would the AxR show?
Mucousal thickening
Dilatation
Perforation
What would the sigmoidoscopy biopsy show?
Inflammed, granular mucosa
Goblet cell depletion
Ulcers
What would the barium enema show?
Loss of haustra
Shortened colon
What criteria is used to assess severity of UC?
Trulove and Witts criteria
- No. of movements per day
What indicates mild UC?
<4 movements
- Few rectal bleeds
What indicates moderate UC?
4-6 movements
- Some rectal bleeding
What indicates severe UC?
> 6 movements
- Rectal bleeding
- Pyrexia
What are the indications for surgery in UC?
Perforation
Massive haemorrhage
Toxic dilatation
What surgery can be done?
Proctocolectomy + terminal ileostomy
