What is the pathology?
A
Atherosclerosis
- Anything reducing the flow of the IMA/SMA

Symptoms of colonic ischaemia?
A
One of the DDx for GI bleeds
- LLQ pain
- Bloody diarrhoea
- PR bleeding
Signs of low blood volume
- Pyrexia
- Tachycardia

Investigations?
A
Barium enema
Endoscopy
MR angiography

What will be shown on the endoscopy of ischaemic colitis?
A
Submucosal swelling
- Thumb printing
- Gangrenous bowel if advanced

DDx?
A
Acute mesenteric ischaemia Crohns Ulcerative colitis Diverticulosis Bowel cancer
this is the DDx for haematochezia

Treatment of ischaemic colitis?
A
Usually supportive care unless extreme
- Nil by mouth
- Antibiotics
- Fluids

Q
Treatment of severe ischaemic colitis?
A
Percutaneous transluminal angioplasty/ endoscopic stenting
Resection of gangrenous bowel
Prevent shock
