Presenting symptoms?
Often non specific
- Dyspepsia
- Weight loss
- Vomiting
What are the signs that suggest incurable disease?
Large epigastric mass
Hepatomegaly
Jaundice
What lymph node would indicate poor prognosis?
Virchow’s node involvement
What skin sign may indicate metastasis
Acanthosis nigricans
Investigation?
Endoscopy
- Biopsy of all ulcers if present
Contrast meal
EUS/CT/MRI for staging
Surgical treatment?
Subtotal gastrectomy
Total gastrectomy w/ Roux en Y reconstruction
What is Roux en Y reconstruction?
The oesophagus is attached to the ileum and duodenum attached more distally once stomach is removed
Where are tumours that require a total gastrectomy?
Proximal 1/3rd of the stomach
What is the use of chemotherapy in gastric cancer?
Increase life expectancy
Palliative pain
What chemotherapy drugs can be used in gastric cancer?
Epirubicin
Cisplatin
5-FU
What is the 5 year survival of gastric cancer?
<10% overall
Improved significantly by surgery
