- I GET SMASHED Idiopathic/Iatrogenic Gallstones Ethanol (alcohol) Trauma Steroids Mumps Autoimmune Scorpion venom Hyperlipidaemia ERCP Drugs

Pathology of acute pancreatitis?
A
Autodigestion from pancreatic enzymes

Symptoms?
A
Severe epigastric pain - And central abdomen - Radiating to back Vomiting Fever

Signs
A
Tachycardia
Jaundice
SHOCK
Rigid tender abdomen
Cullens sign
- Grey (bruised) umbillical area
Grey turners sign
- Grey (bruised) flanks

Investigations
A
- ABG
- Bloods (FBC LFTs)
- EUS
- AxR/MRI
- ERCP if suspected cause gallbladder or if worsening LFTs

Findings
A
- Serum amylase up
- Serum lypase down after 24hrs

Surgical management?
A
ERCP gall stone removal

Pharmacological management?
A
- Analgesia
- Antibiotics
- May require insulin

Other management?
A
- NG tube
- IV fluids
- Hourly obs
- Daily FBC, U&Es and LFTs

Complications?
A
Pancreatic necrosis Pseudocyst Shock Abscesses Recurrent oedematous pancreatitis

Treatment of pancreatic necrosis/pseudocyst?
A
Laparotomy with parenternal nutrition

Treatment if recurrent oedematous pancreatitis?
A
Pancreatectomy considered
