What is family understanding of transition
Identify who are key teams (medical + AH) that will need to be involved longer term
Think about what young person might do to replace school
- Uni/TAFE
- Day programs
- Disease specific peer
groups
- Socialising
Get young person to have time alone in appointments/attend appointments alone
Involve transition team if one exists
Period of overlapping appointments
Medical handover
Tie up loose ends
