Management Poor Growth

A
What is the growth potential?
- Mid parental height
- Bone age
- Centile, trajectory 
History:
- Intake, malabsorption, energy use
- ?reflux contributing 
Consider cause
- Iatrogenic/intrauterine, skeletal dysplasia/scoliosis, nutritional, chronic disease/chromosomal, endocrine (hypothyroid, cushing, GH deficiency)

Investigations

  • Anthropometry
  • FBE, UEC, LFT, TFT, coeliac screen, ESR, iron
  • Sweat test
  • GH/cortisol
  • IGF1
  • Stool - fat globules, crytals
  • Urine MCS/metabolic
  • Cerebral imaging
  • Karyotype

Treatment - involve speech path/dietician

  • Offer solids before liquids
  • Relaxed meal times
  • Treat reflux - position, thicken, PPI, fundoplication
  • Manage chronic disease
  • Bolster diet
  • Consider GH therapy
