It is a viral skin infection caused by a large DNA pox virus with an incubation period of 2 to 7 weeks. The lesions consist of multiple pearly round papules measuring 3 to 5 mm in size with central umbilication.

There is lobular hyperplasia of epidermis into the dermis resulting in a cup-shaped lesion. The basal layers appear normal, but the keratinocytes of stratum malpighii become enlarged and acquire eosinophilic inclusion bodies - Henderson-Patterson bodies that contain virus particles.
