When is diarrhea considered chronic?
> 2 weeks
List Extra-GI causes of Diarrhea
**1. Acute pancreatitis
2. EPI
3. Hepatic insufficiency
4. Renal failure
5. Hypoadrenocortism (Addisons)
6. Hyperthyroidism (cats)
List GI causes of diarrhea
- AHDS
- Acute colitis
- IBD
- PLE
- Intussusception
CS of Acute hemorrhagic diarrhea syndrome in dogs?
- Sudden onset of bloody, watery diarrhea
- Marked elevation in PCV (>50%) due to dehydration
- Vomiting > 10 hours prior to diarrhea onset
Tx for Acute hemorrhagic diarrhea syndrome in dogs?
- 1st step is IV fluids!! Correct and avoid hypovolemic shock
Viral cause of SI diarrhea in young dogs?
Parvo
Viral causes of SI diarrhea in cats?
- Feline parvo / panleuk
- Coronavirus
List bacterial causes of SI diarrhea
Campylobacter jejuni, Salmonella spp, E.coli
Tx plan for a patient with SI diarrhea and a positive culture for salmonella?
Only tx with Abx if systemic signs of infection or immunosupression!! Wide resistance reported
What bacteria is linked to granulomatous colitis?
E coli
When are fecal culture and PCR testing recommended?
- Colitis and signs of systemic inflammation
- Outbreaks of acute diarrhea
- After histo evidence suggestive of infectious enteritis
Diarrhea without systemic CS… does NOT warrant a fecal culture or PCR
List breed specific enteropathies
- Boxers and frenchies with granulomatous colitis
- Wheatens with PLN/PLE
- Gluten sensitivity in irish setters
What is food responsive chronic enteropathy?
- Younger dogs that resolve with diet change
- Resolution in 2-4 weeks on novel protein or hydrolyzed diet
Tx plan for a patient with 3 weeks of CS consistent with chronic enteropathy?
- Food trial
- If fails, do immunosupressive Pred trial
- Consider Antibiotic responsive CE if evidence of systemic infection
- B12 injections
- Probiotics
- Fecal transplant
