ankylosing, IBD, psoriatic, reactive arthrtis (and lyme)
ankylosing
men, 95% HLA, 3rd decade, stiffness decreases with activity, gradual spinal fusion, acute iritis, risk: genetic and environmental
IBD
with or without related bowel activity. 70% HLA
Psoriatic
closest resemblance to RA, asymmetric oligo, prominent enthiasis, sausage diget. 5% psoriatic pts
reactive arthritis
antigen in joint but not septic, post urogenital/GI infection (gohnorhea or salmonella). oligoenthesis 1 wk post infection, most resolve, HLA 50%
treat all besides lym
NSAIDS, anti TNF, biologics
Lyme arthritis
stage 3 of untreated infection, 6+ or later, 50%, known cause for chronic . HLADRB1. treat: oral ABX
lyme clinical
intermittent, recurring attacks. mild. 1 joint, usually knee.
lyme path
active infection in synovium> T cells activate> RA like lesion. culture will be sterile but PCR shows infection
rare chronic lyme arthritis
no infection… debris?
Hla haplotype?

Psoriatic, Ankylosis
Hla b27 association
Reactive 50, Ibd 70, psoriatic weak, Ankylosis high
