ortho spine Flashcards
what is the axial load on the spine?
gravity
what mechanics are on the annulus
tensile stress (TI collagen)
what mechanics are on the pulposis
compressive strength, (TII collagen)
what position is the highest pressure on the spine?
sitting, leaning forward
facet innervation
dorsal medial branch
annulus inervation
recurrent meningeal nerve
lateral vs paracentral herniation
lateral- superior nerve, paracentral, inferior nerver
herniation treatment
usually improves on its own, MRI if not or neuro symptoms. surgery if unstable or caudal equina
Clauda equina
severe canal stenosis, saddle anesthesia, bladder/bowel incontinance
neuro claudication
postural, walking/standing worse, normal pulse, bike is fine
vascular
standing is better, abnormal pulse, bike worse
red flags with back pain
constitutional changes, cancer, infection, bowel/bladder symptoms, near issues, trauma
L1/L2 myotome
hip flexion
L3/4 myotome
knee extention
l4 myotome
ankle dorsiflexion (tib. ant)
