What is retinal detachment?
Where retina separates from choroid underneath
-Sight threatening emergency
Cause of retinal detachment?
Usually due to retinal tear
-This allows vitreous fluid to get under retina & fill space between retina & the choroid
What does the outer retina rely on?
Blood vessels of the choroid for the blood supply therefore sight threatening
Risk factors for retinal detachment?
- Posterior vitreous detachment
- Diabetic retinopathy
- Trauma to the eye
- Retinal malignancy
- Older age
- Fam Hx
Presnetation of retinal detachment?
Painless condition
- Peripheral vision: shadow coming across vision
- Blurred or distorted vision
- Flashes& floaters
General management of retinal detachment?
Detailed assessment of retina
-Aim to reattach retina & reduce traction or pressure that may cause it to detach again
Management of retinal tear?
Laser therapy
Cryotherapy
Aim of retinal tear management?
Aim is to create adhesions before retina & choroid to prevent detachment
Retinal detachment management?
- Vitrectomy
- Scleral buckling
- Pneumatic retinopathy
What is vitrectomy?
Removing relevant part of vitreous body and replacing with oil or gas
What is scleral buckling?
Involves using silicone buckle to put pressure on outside of eye (Sclera) so that the outer eye indents to bring choroid back & in contact with detached retina
What is pneumatic retinopexy?
Injecting gas bubble into V. body & positioning patient so gas bubble creates pressure that flattens retina against choroid and closes detachment
Presenting complaint for retinal detachment?
Sudden painless loss of vision
Symptoms of retinal detachment?
Persistent flashing lights
Floaters
Dark shadow in peripheral vision
Relative afferent pupillary defect (RAPD)
Causes of retinal detachment?
Myopia Previous detachment Fam Hx Marfan's syndrome Trauma
