What is episcleritis?
Benign self-limiting inflammation of the episclera
What is the episclera?
Outermost layer of sclera, situated just under conjunctiva
Who usually gets episcleritis?
-Young & middle aged adults
What is episcleritis associated with?
-RA
-IBD
(Both inflammatory disorders)
-Gout
Presnetation of episcleritis?
Acute onset unilateral symptoms:
- Typically not painful
- Segmental redness (usually red patch in lateral sclera)
- Foreign body sensation
- Dilated episcleral vessels
- Watering of eye
- No discharge
- Nodules may occur
Management of average episcleritis?
- Usually recovers in 1-4 weeks
- Mild cases need no treatment
- Lubricating eye drops can help symptomatically
- Simple analgesia, cold compress & safety net device
Management of severe episcleritis?
SELF-LIMITING
Topical/systemic NSAIDs (naproxen)
Topical mild steroid eye drops
Episcleritis is caused by infection. True or false?
False, not usually caused by infections
What is episcleritis associated with?
Gout
Differentiating episcleritis from scleritis?
Phenylephrine drops
-causes blanching of blood vessels in episcleritis not scleritis
